How a Care Plan Works

Real support, built around your needs

Your first visit with Melissa includes a full nursing assessment and a relaxed, in-depth conversation about your medical history, current needs, and personal goals. She’ll listen closely, ask thoughtful questions, and begin mapping out a care plan that fits your daily life. This initial visit forms the foundation for everything that follows, making sure your care is grounded, practical, and tailored to you.

Some clients choose a set number of visits per month — for example, four appointments during a post-surgical recovery period. Others prefer an hourly arrangement that can flex week to week. Melissa will walk you through the options so you feel informed and in control.

Care plans may include:

  • Medication management and education
  • Wound care, dressing changes, or drain monitoring
  • Chronic condition support (like diabetes or CHF)
  • Dementia and Alzheimer’s safety guidance
  • At-home lab draws and vitals monitoring
  • Preventative visits to avoid ER trips

Every visit includes professional documentation and, when needed, coordination with your doctor or loved ones. Melissa can also accompany clients to medical appointments, order supplies, and help navigate home health decisions with clarity and compassion.

Simple, Transparent Pricing

Clear rates, no surprises — and always a conversation

Worthy Care offers flexible pricing based on the kind of support you need. Every client begins with a comprehensive first visit, which includes a full nursing assessment, medication review, and personalized care planning. From there, Melissa will recommend a plan that matches your situation, whether it’s a short-term recovery or ongoing care.

Common pricing options include:

  • Hourly care: $75 to $300 per hour, depending on the complexity of services
  • Monthly care plans: Starting at $400/month for four visits, with room to customize

Melissa does not accept insurance at this time, but payment is simple and secure. You can use major credit cards, Venmo, PayPal or Clover. You’ll always know the expected costs before scheduling and you’ll never be charged for services you didn’t agree to.

Worthy Words ~ Testimonials Highlighting Compassionate, Professional Nursing.

“I wanted to express how impressed I was with the level of care I received from my nurse Melissa at my own home. I am forever grateful she was able to draw my bloodwork at home after my doctor ordered labs. I was just too sick to leave the house. She delivered the labs as well as communicated with the on call doctor at my PCP for me. What a great service. This just doesn’t seem to be available through my health insurance.”

~Geoffrey P.

“Melissa’s care for my mom during hospice…all I can say is she went above and beyond to care for not only my mother, but the entire family so we could all understand what was happening. It takes a special person. Highly recommend her nursing abilities.”

“My family lives across the country. I have been looking to hire a nurse to check in weekly on my parents, no insurance seems to cover this service. I found Melissa through a recommendation. They are stubborn, and don’t want help. I don’t know how she worked her magic, but now she is visiting them weekly. Vital signs checked (one has a pacemaker and the other has heart failure), full assessments, making sure their medications are filled and taking them correctly…a great system she set up just for them!”

~Patricia D.

“Melissa cared for me as a travel nurse taking care of my chronic wound. She advocated and really stood up for me to get the most appropriate care. Using a wound vac, and I am now completely healed. I was not an easy patient.”

~Ramona M.

“I am chronically ill. It is hard to speak for myself to get the care I really need. She patiently explained what to expect, helped coordinate all of my appointments, made sure all of my pre-op requirements were met, and spoke to doctors offices and pharmacies at length for me. She ordered supplies I did not even know were possible to get. This kind of nursing care is hard to find.”

~David R.

“I was having trouble with a new catheter after a procedure I had. I started having pain and did not want to go to the ER as I know how expensive it costs, even with insurance. Melissa was able to make a visit, provide care, catheter was clogged! I actually feel comfortable with doing this on my own now. My doctor was updated by her as well and made me a follow up appointment. Highly recommend.”
